The STUDENT must complete this form in his or her own handwriting. It should be completed in PEN not pencil or typed. We suggest your student write a rough draft first, review and edit content to questions prior to completing the official application for submission.

The parent(s) or guardian(s) should complete this form.

TEACHER REFERENCE FORM


After the STUDENT STATEMENT and PARENT OR GUARDIAN STATEMENT are completed, the student must give the complete packet to one of his/her current academic teachers to complete and forward to the principal for submission to our office.

What if I want to apply but find the application difficult?
Call (904) 390 -2133 with any questions, or make an appointment to meet to receive advice and encouragement.

Where will the program be held?


This program will be held in following four High School locations: Mandarin, Sandalwood, Lee, and Raines.

Is transportation provided?
Transportation will not be provided; qualified applicants may receive a city bus pass.

Is it difficult to get accepted into the program?


There are many more qualified applicants than spaces available. The Super Summer Bridge Academy Admissions Committee attempts to select a student body that represents Duval County Public Schools diversity. Admission to the program is like a contest rather than a competition: something you take a risk and apply to attend, not something you win or lose.

What type of students does not get accepted to the program?


The same type of students who get in. All applicants sound wonderful. The Admissions Committee enjoys reading all the applications. However, we seek serious students who like to try new things, have a sense of humor and good work ethic, and would also contribute to our Super Summer Bridge Academy experience.

Any important advice?


Remember that applying to Super Summer Bridge Academy is like applying to a contest. You may or may not be accepted, but it is always worthwhile trying out for a new adventure.

Who should submit the packet?


The completed STUDENT STATEMENT and PARENT OR GUARDIAN STATEMENT should be given to one of your current teachers for completion. The teacher will submit the completed packet on your behalf to your school's principal. The principal or his/her designee will fax the completed applications, and then mail the original through the school mail to the address below.

I understand that the four week 2013 Summer Session runs from June 17th through July 12th, Monday through Friday, 8:00 a.m. to 3:00 p.m. I also understand that during the summer session my child will be expected to attend at least 13 of the 19 days as well as a follow-up summer session during the school year. I understand that in addition to the summer sessions and the school year follow up session, there may also be periodic field trips and enrichment activities my child will have an opportunity to attend. The Super Summer Bridge Academy is free academic enrichment programs open to all current DCPS Grade 8 students. We are seeking children with the ability to do well in school and who may not be performing at the full potential of their academic capacity. The program is intense and challenging. It involves critical thinking, collaborative group work, and problem solving skills. The selection committee is committed to selecting a student body reflecting the diversity of Duval County Public Schools.
