NEONATES BORN TO MOTHERS WITH THYROID DISEASE

Hypothyroid Mother If well controlled Hashimotos Thyroiditis, complete routine newborn screening. If hypothyroidism is untreated or uncontrolled during pregnancy, infant is at risk for hypothyroidism. Check a Free T4 and TSH. If mother develops HYPOthyroidism during treatment for HYPERthyroidism (i.e. Graves - thyroid ablation, removal, or medical), the infant is at risk for developing hyperthyroidism secondary to maternal antibodies see below. Hyperthyroid mother Obtain Free T4 and TSH. If normal, reevaluate infant for symptoms of hyperthyroidism at 5-7 days of life. Repeat Free T4 and TSH at 7-10 days of life. If labs are abnormal consider Endocrinology consult. Note: Maternal medications such as PTU/Tapazole may persis in the infants system for 7-10 days while maternal antibodies may

persist up to 3 months resulting in delayed symptoms of hyperthyroidism.
