DEED POLL
By this deed I, the undersigned PALLAVI GARG previously called hereby solemnly affirm and state as follows :1. That for and on behalf of my self and my husband and children and remitter issue I wholly renounce\relinquish and abandon the use of my former name \surname of PALLAVI MITTAL and in place thereof I do hereby assume from this date the name \surname PALLAVI GARG and so that I and my Mother & remitter issue may hereafter be called, know and distinguished not by my former name \surname, but assumed name \ surname PALLAVI GARG. 2.
