    Aritra de was diagnosed with tubercular lymphadenitis on 26th April 2012 at the Apollo Hospital, Bhubaneswar and subsequently started taking medications. At the time of the re-opening of the semester classes of the 7th semester I was down in bed with tuberculosis and subsequently had two surgeries.     Aairs) NISER, Bhubaneswar Respected Sir Subject: Application for Semester Leave I was diagnosed with tubercular lymphadenitis on 26th April 2012 at the Apollo Hospital, Bhubaneswar and subsequently started taking medications. At that time, the end -semester exams were going on for my sixth semester. After the end-semester exams, I went home and continued my medication. During the time of the re-opening of the semester classes of the 7th semester I was down in bed with tuberculosis and subsequently had two surgeries. I have herewith enclosed my case history. I hereby apply for semester leave for the seventh semester.
