Please be informed that I was supposed to leave for employment in Alberta, Canada on June 4, 2013 as a Cashier under employer, SUDS AND SPIRITS LIQUOR STORE/Don Sylvestre. However, due to loss of my passport with corresponding Canadian Visa during my stay in Taiwan, I may not be able to catch up with my scheduled deployment. In lieu hereof, I am respectfully requesting your kind consideration and approval for urgent reissuance of passport for further facilitation/reissuance of Canadian visa as well as facilitation of travelling documents with Philippine Overseas Employment Administration (POEA) thus pursue my deployment/employment in Alberta, Canada. In support of the above request, herewith attached following documents confirming the above for your immediate reference: 1. Copy of Affidavit of Loss duly notarized in Taiwan; 2. Copy of Police Report; 3. Travel Documents used on my returned from Taiwan to Manila, issued on May 20, 2013 by Manila Economic Cultural Office Taichung Taiwan 4.
