INCIDENTS, NEAR MISS AND SENTINEL EVENTS IN THE HOSPITAL Incidents: An injury related to medical management in contrast to complications

of disease Example: Wrong procedure (wrong site, wrong patient, wrong procedure) Retained foreign body left at surgery Blood or transplant organ incompatibility Transmission of an infectious agent to a patient Administering incorrect medication or dosage In-hospital trauma (falls, burns) Deep venous thrombosis or pulmonary embolus without appropriate prophylaxis Surgical site infection without indicated antibiotic prophylaxis Pressure ulcers Catheter-acquired infections Near Miss: Any incident that had the potential to cause harm, but didn't due to timely intervention and or luckor chance. Sentinel Events: A sentinel event is an unexpected occurrence involving death or serious physical or Psychological injury or the risk thereof. Serious injury specifically includes loss of Limb or function. Such events are called sentinel because they signal the need for immediate investigation and response. Example:

Suicide while a patient is under 24/7 surveillance Unexpected death Infant abduction or discharge to the wrong family Rape Haemolytic transfusion reaction due to mismatched blood
