Dear Sir, Upon receiving notification from The Open Government Unit on 27th August that your personal details had been inappropriately disclosed, I immediately undertook to identify the source and how this had occurred. I regret I have been unable to establish the exact circumstances, or how this has happened. I can however, assure you that steps have been taken to ensure those involved in the process are aware and have had their responsibilities reconfirmed to them, in order to ensure this does not happen again.
