Textbook Release Form September 3rd, 2013

Dear Parents and Guardians, Welcome to another wonderful school year. The purpose of this letter is to inform you as well as your student on the procedure for obtaining all of their school core subject area textbook as well as a contract between you and the school. This year we have purchased numerous new textbooks for the students and the students will have the privilege of taking some of these textbooks home to complete any assignments under the following conditions. First of all, textbooks are going to be issued to students only if they have this signed contract on file. Each student will be assigned a number, which is recorded. Afterwards, the textbook will become the responsibility of the individual students. If a textbook is lost, the textbook must be replaced at the students expense according to the individual price of the book. In addition, damage fees for misused and abused textbooks will be assessed at the conclusion of the school year. If the textbook is either not returned or the fee has not been turned in to Crescent Academy, the school will withhold any and all of their report card, grades, and certificates until the fee or the book is turned in. Please sign and complete the bottom portion of this form and return to the students homeroom teacher by Friday, September 6th, 2013. Here is a list of the textbooks being used for the Crescent ELITE Program as well as the price for the particular textbook: Reading Textbook: Individual Novels - $10.00 for each novel Mathematics Textbook: Course Books - $75.00 (May change based on new book price.)
