Print onto card stock, and cut out. To make the collar, cut along the solid line in the center, and fold down along the dotted lines. Fold tabs at the edge of each strip, and attach additional strip to the collar with double-sided tape. After placing fudge in the container and closing the lid, use double-sided tape to attach collar to the top of the box. This template is perfectly sized for a 4-inch-square box, but to create wrapping for a larger container, simply print additional strips to attach to collar.
