What is Graduation Transitions?

The Graduation Transitions Program is to help and assist students make the transition from secondary school to post-secondary or their future endeavours. All Grade 10 12 students will be required to meet prescribed learning outcomes in three areas: 1. Personal Health 2. Community Connections 3. Career and Life Students will begin this program in Grade 10 and will have until the end of their Grade 12 year to meet these requirements. Completion of these learning outcomes plus and exit interview (presentation) will be noted on their transcripts as RM (Requirement Met) for Graduation Transitions, and Grade 12 students will be awarded four credits. In Planning 10, you will receive a folder to collect all required components of the GT program. This folder will be kept by the Graduation Transitions coordinator and as you complete various components, it will be kept in your folder. In Grade 12, during your Exit Interview, you will receive your folder and present everything you have completed. At the conclusion of your interview, you will return all completed components (your folder) back to the Graduation Transitions coordinator for final evaluation. Personal Health (Can be completed in Grades 10, 11 or 12)


150 minutes of Daily Physical Activity each week in grades 10, 11 and 12 complete and submit Declaration Form each year Personal Health Reflection Questions *To be completed as part of Planning 10

Community Connections (Can be completed in Grades 10, 11 or 12)


30 hours of work experiences and/or volunteer work fill out log and provide supporting documentation (e.g. paystub, letter, Co-op hours sheet) Reflection Questions

Career and Life (Completed in Grade 12 Year)
