Condemnation procedure of hospital


1. User department should prepare the condemnation letter (format given) and get the signature of the Head of the Department. 2. User department should sent the condemned item to the condemnation committee / Auditor with the condemned items for verification. 2.1In case of equipments, justification report for irreparable condition of the equipment should be obtained from the service provider and should be enclosed with condemnation letter. 3. Condemnation committee / Auditor should physically verify the items as well as the justification report given by the service provider and approve the condemnation. 4. The user department should hand over the condemned items to stores and have to get the acknowledgement from the stores. 5. Condemnation letter should be approved by G.M or any other authorized person. 6. Stores should keep the condemned items in the scrap yard for disposal. Note: 1. Justification report is applicable for Medical and Non-medical equipments. 2. Condemnation committee consist of Material Manager, Maintenance Engineer(in case of Non-Medical equip.), Bio-Medical Engineer (in case of Bio-Medical equip.), G.M (Operations)
