Internship Weekly Report Format Note: Weekly report should be submitted on a specific day of the week. That day will be mutually decided by the faculty advisor and the intern. This weekly report should be composed and written in M.S. OFFICE (Microsoft word, Excel etc) and sent to faculty advisor by way of an attachment to e-mail. The format of attached document: title should look like this WR1Name if student. Doc. the next weekly report should be titled as WR 2, and so on until reach weekly report number 06 and/or 08 for the internship program. Below is an example of weekly report Please note the following part of the report: a) Personal information-WR1 along with your name, semester, course, name of the organization and the calendar date for the week. b) Report must include the number of hours, date and the day of the week in bold.
Remember you will have a space in the last part of the form to give your feedback and reactions to the week. Note: Keep these report in safe custody and turn in at the end of the semester. Suggested Outline for Presentation The presentation should highlight the major section of your written Department of Management Sciences (DMS) Internship Report. The presentation should be in Multi Media and follow the suggested format below: 1. Title (1 slide): your name, the host organization, and your job title. 2. Introduction (1 slide): Mission statement or background of the host organization. 3. Objective of Internship (1 slide) 4. Methods, Skills, and Technology (3 to 4 slides) 5. Results (3 to 4 slides) 6. Conclusion (1 to 2 slides) 7. Graphics (1 to 5 slides): Include photographs of you working in the field, maps, report covers, diagrams, and other photographs as necessary to augment your presentation.
Internship Policy
Guide Lines for Internship Program To ensure the quality and consistency, the faculty advisors and interns are responsible to follow the Internship guide lines in letter and spirit. Criteria for Internship Approval 1. Internship being equivalent to a 3 credit hour course. Therefore, it requires 6-8 weeks of practical work in an organization of national or multinational repute. The assignment of internship organization requires the approval of the faculty advisor. No student has the right to choose an organization without the formal written approval of the faculty advisor. Internship involving potential conflict of interest is not allowed e.g. working in a small family business would not be acceptable. Practical work requirement of internship program is acceptable in an organization where a student is already employed subject to the approval of the Head, Department of Management Sciences. In this regard students are directed to submit an application along with the profile of Employer/Organization. However, to earn the full credit for internship, such students will be required to fulfill all other requirements of the evaluation criterion. Any extension in the period of internship will require an approval from the Head, Department of Management Sciences on recommendation of the faculty advisor.

The JPC will also solicit the CVs of all potential interns and to prepare a database. The JPC will assign the interns various available organization while matching the career goals of the interns with the needs of the organization. The JPC will monitor the over all progress of the Internship program.
Role of the Intern: 1. 2. 3. 4. 5. 6. 7. The intern is required to join the assigned organization as per given schedule to the respective faculty advisor. After joining the organization, the intern is required to submit a copy of the joining report to the respective faculty advisor. The primary supervisor of the intern is the on-site supervisor. Interns shall mail one page weekly report to the faculty advisor. The report will consist of the activities performed and problem faced during the week. The student will be responsible to inform faculty advisor about completion of his/her internship period. A draft of internship report as per formal format will be submitted within two weeks to the faculty advisor after the completion of internship. After satisfactory evaluation of the report by the faculty advisor, intern must submit three hard bound copies and one soft copy of the internship report to the faculty advisor. If intern fail to earn passing grade then he has to re-register himself with the HOD office for repeating of internship against the course fee.

Evaluation:
Three credit hours maximum will be earned by the internship. Evaluation of the students performance during the internships will be coordinated among the student, faculty advisor of the internship, and on-site supervisor, Evaluation should consist of a combination of measures.
