OCEAN VIEW CHURCH CONSENT AND RELEASE FORM FOR PIPELINE I, the undersigned parent or guardian, hereby consents to my child, ______________________, who is _______ years of age, participating in the activities connected with Pipeline Ice Skating and Pizza @Incredible Johns, an activity sponsored by Ocean View Church on Saturday, September 28, 2013. I certify that my child is able to participate in any and all activities associated with this event, excluding: _______________________________________________________________________ Telephone Numbers Where I May Be Reached In an Emergency: _______________________________________________________________________ OCEAN VIEW CHURCH ARBITRATION AGREEMENT This agreement represents a voluntary understanding between the child/family and the sponsoring church/and or school (hither to referred to as ministry) which both parties have agreed conforms to all Scripture regarding the matter of going to law against a brother in Christ. The ministry and we (I) each agree and understand that we choose binding arbitration instead of litigation to resolve any dispute between us. The ministry and we (I) each understand that we have a right or opportunity to litigate disputes through a court, but we choose to resolve our disputes through arbitration. All of the parties voluntarily and knowingly waive any right they have to a jury for any litigation or cause of action arising out of the partys relationship to one another. It is the responsibility of the ministry to secure three independent Biblical pastors who will serve as the arbitration panel. Both parties agree that the decision of the arbitration panel is final.
