Illinois Licensing Information

RESIDENTS AND NON-RESIDENTS:

Complete the licensing application and submit it back to Pre-Paid Legal Services, Inc.,
P.O. Box 1443, Ada, OK 74821, along with a $50.00 check made payable to Pre-Paid
Legal Services.

The State of Illinois will not accept P.O. Box addresses. Please give a physical address
so that your application can be processed without delay.

Once you receive your license, you may begin marketing.

NOTE:
The Illinois license will expire on December 31 every year. You will be notified of the renewal.
