    (Only the adults needed to meet Volunteer Essential standards should be considered chaperones.)

    Chaperones are free

    No tag-a-longs (other family members), please!

    Make checks payable to SU 10-12 You can register at the event!


    Complete Registration includes: Payment for each attendee Health History Form for each attendee Permission Slip for each girl

    Bring! Girls 1. A costume! 2. Clothes for the weather 3. Closed-toe, walking shoes 4. A flashlight for the maze 5. Extra cash for the Forrest Hall Farm store if desired they have apples, pumpkins, chrysanthemums, preserves, Christmas ornaments and more! Troops 1. A permission slip for each girl 2. A Health History form for each attendee 3. Cash or Checks made out to SU 10-12 4. Blankets for the Hayride if cool outside.
