I am referring my patient, Audrey Smithe, to your clinic for the purpose of seeking highvelocity, low-amplitude manipulation treatments to her cervical spine at vertebras C6-C7. Ms. Smithe has been a patient of mine for one year now, and recently had a fall resulting in a lateral and posterior herniation of disc C6 in her cervical spine. This led to a radiculopathy of her cervical spine at C7. Up until this point Ms. Smithe has had a normal heath history; she has not had any major injuries nor outstanding health conditions. She has been seeing me for musculoskeletal purposes (deep tissue massage and trigger point release) focused in her lower back, anterior hip, and legs. Post fall, Ms. Smithe presented with symptoms of lateral neck pain, right arm pain radiating distally, and tingling sensations within the right hand. Upon assessment, I discovered that she tested positive for Spurlings Test and Foraminal Compression Test, both of which elicited her symptoms.

If you agree to take Ms. Smithe on as a patient, I will adjust my treatment plan accordingly. I am planning to do myofascial work and musculoskeletal techniques to reduce the amount of fascial restrictions in her entire neck, as well as reduce the guarding muscle resting tension in both her anterior and posterior neck muscles (notably sternocleidomastoid, scalenes, splenius cervicis, and upper trapezius). I look forward to your response and I hope we may continue to correspond in regards to this case to discuss treatments in more depth and to collaborate on the progress of Ms. Smithe together.
