Wound Care Definition of wound care:

It is the application of a protective covering over a wound by using a sterile technique.

Objectives:
1- To prevent hemorrhage 2- To absorb drainage if present. 3- To provide or maintain high humidity of the wound 4- To prevent infection

Dressing
Equipment:
1- A sterile dressing set includes: a) Forceps b) Scissors c) Sterile gauze d) Sterile basin 2- Sterile gloves 3- Sterile Normal Saline 4- Cleansing solution 5- Antiseptic solution 6- Clean disposable gloves 7-Tape ties on bandage as needed 8- Water proof bag 9- Extra gauze dressing 10- Draping sheet 11- Antiseptic ointment if prescribed

1-Identify the patient 2- Review physician order for dressing change procedures 3- Prepare equipment at the bed side 4- Explain the procedure to the client 5- Position the patient in comfort position and put draping sheet to expose only wound site 6- Place disposable bag within reach of work area 7- Hand washing 8- Wear disposable gloves 9- Remove tape, pull parallel to skin, and pull toward dressings. remove remaining adhesive from skin 10- Carefully remove gauze dressing, one layer at one time 11- Dispose soiled dressing & remove gloves. 12- Put on sterile gloves. 13- Open sterile dressing set. 14- Open bottle of antiseptic solution&pour into sterile basin I

blood, odor, patient tolerance, any abnormalities if found observe and take sample if needed promote early treatment
