.-A Better Life Program Manager Orientation Introduction: Good Morning! My name is program manager for whom you will be serving. I would like to take this opportunity to tell you a little bit about my Dackground......... ‘As an HTS you have a vital role in the success of the people that you serve, Itis with your teaching, training and mentoring that people reach their full potential 1. The first topic we will be covering are Data Books and the individualized forms. Data Books are in every individual’s home. Itis very important that al forms in the book are completed before leaving shift. Without proper documentation, we cannot justify your services. You play the largest role in this; we depend on you, ‘a Sign-in log: Please insure that you sign this when reviewing or providing documentation in the Data or IP Book. a Demographics: Provides you with information on the individual you serve. (D.OB., address, case number, PM, guardian, ete...) 2 Recreational and Leisure Calendar: To document individual's activities at home and in the community. 2 Progress Notes: Individual specific. IP will specify if it is every 30min, one- hour or two hours. a Outcomes: Outcomes are derived from the IP and are individual specific. These are the goals set by the team for the plan of care year. @ Datacollection: Individual specific. This could include: Intake and output, B/P, weight, positioning chart, food log or sleep record, etc... 2 Continuous Medical Record (DDS-36): Record kept of lab work, med changes, incident report s and doctor’s visits. @ Doctor’s Orders (DDS-5): Form that is taken to each doctor’s visit; it provides us with vritten doctor’s orders for the individual, 2 Med count/log: Completed when coming on shift, administering a medicati receiving a medication (from pharmacy) and before leaving shift a Fire and Tomado Drills: Performed quarterly per DDSD policy and insures that the individnal and staff are properly prepared for this emergency. ‘CADocaments and Seaingssbonbars\CocalSeting\Temporary Intemet FileAOLICT3 Program Manager In Servize doc vA Better Life Program Manager Orientation 2. Miscellaneous Forms and what they are used for. @ Staffing Pattems: These are filled out by the PM and placed in the home. They will insure you of your scheduled shifts. Timesheets: Time runs from 12midnight Friday to 12midnight Thursday. Print your name clearly. Any training is signed in on the bottom line of that day. Print your name and class on that line a Contact Reports: Are used to provide documentation of conversations or incidents that need to be reported. © Incident Reports: Incidents, whether, physical, behavioral or medication are to be reported in a timely manner. This form filled out and into your area office by 8a.m. the next business day ifnot sooner. Program Manager or Program Manager On-Call should always be notified if an incident form is being utilized. @ Mileage Logs: Mileage is turned in weekly. Print name clearly, write transported at the beginning of each description, whom you transported, where to and why. Odometer reading must be documented also. Ex. Transported H.C. from 3521 S. 24" St, Muskogee to 501 N. Maine, Muskogee (Shop at Arrowhead Mall) and then returned to 3521 S. 24" St., Muskogee. ' a Petty Cash Accountability Sheets: Petty cash must be counted at shift change and documented. Any discrepancies must be reported to PM or PM On-Call. When purchasing items for the individuals with petty cash, please insure that you sign or initial the receipt and document it on the form. 3. Specialized Programs, instructions and/or in-services pertaining to each individual. | a Transporting: 1. Employees are required to maintain a safe, reliable vehicle that meet applicable state and federal guidelines at all times during employment, -- 2, Allvehicles should be suitable for any transportation need... ee 3, Allemployees must have current insurance coverage and valid driver's license at all times during employment. 4, All operations staff who work in the field are required to transport individuals to and from activities in the community. 2 Behavioral Program: Individual specific. Any employee who will be absent or tardy must notify your PM four hours prior to your scheduled time of arrival. A call to your PM does not necessarily excuse an absence or tardiness. Excessive tardiness will result in disciplinary actions up to and including termination. ‘a Call in Policy: When calling in for a shift, you are to page your Program Manager or ifit js on the weekend or after five the PM On-Call. 1. Your absence of tardiness affects the service delivery of our organization. Ail employees are expected to maintain a good attendance records. 2, Any employee who will be absent or tardy must notify your immediate supervisor four hours prior to your scheduled time of arrival, A call to your immediate supervisor does not necessarily excuse an absence or tardiness. Any employee missing their scheduled shift with out proper notification will be subject for disciplinary actions up to and including termination. 4, All absences due to medical reasons may require a Physician’s note upon returning to ‘work based on the discretion of the supervisor. 5, Employees who are absent from their work assignments for unknown reason for three consecutive scheduled workdays are automatically considered to have voluntarily resigned their positions from this agency and will be advised such if they subsequently report to work. a Dress Code: Appropriate dress is required by Bios at all imes. Appropriate dress defined: ‘2. Must wear appropriate undergarments, i. no halter-tops, tube tops, tank tops with thin straps. b. No bare mid-drifts c. Shorts, skirts, dresses, etc. must be no shorter than the length of a dollar bill laid vertically to the knee, long way, starting at the hem and ending at the top of the kneecap. d. Clothes stich as tee shirts, hts, ete. must not represent any questionable products or morals (such as promoting drags, drag paraphernalia, cuss | words, sex, etc.) @. Shoes must be wom at all times.
