PROCEDURES AND RELATED INFORMATION

1. Application forms are available at the offices of DE Phones/Telegraph/Customer Service Centers and PTCL web site.
2. Application duly filled up be deposited at any Customer Service Center. Incomplete application may delay the process.
3. Separate applications are required for different addresses.
4. Following documents are required to be attached with the application:

A) INDIVIDUALS B) CORPORATE CUSTOMERS/COMPANIES


i) Ownership / Lease deed i) Ownership / Leased deed
ii) Customer’s I.D or Passport No. ii)
Certificate from Registrar of Firms / Securities & Exchange
(Copy) Commission of Pakistan.
iii) Authority letter of Company to sign Documents.
Note: Fresh documents are not needed if additional telephones are required at the premises already having a phone
in the same name.

5. PRIORITIES:

i) Self-Finance Scheme (SFS).


ii) For availing “Priority” in the following cases, registration/certificate/documentary proof would be required;

Hospitals / Maternity Homes, Doctors, Homeopaths, Hakims, Nurses / Midwives, Engineers, Architects,
Advocates, Red Cross Societies & Clubs, Darul Amman, Software Exporters / I.T. Experts, Air Companies, Shipping
Companies, Chartered Accountants/Auditors, Chamber of Commerce, Share & Stock Brokers, Schools & Colleges,
Cinema Houses, Banks, Accredited Press Correspondents / Journals and Newspapers / Magazines, Recognized Merchants
Associations, S.M.Es (Small Medium Enterprises), Exporters, Important Business Houses, Industrial Units / Residence
established by Overseas Pakistanis, Petrol Pumps and CNG stations, Public Institutions.
Persons paying Rs. 10,000/- or more as annual Income T ax (Individuals) – Rs. 50,000/- per Annum (Companies).

A) Please insist for code barring which is provided free of cost to safeguard against any misuse.
B) It is your national duty to pay bills on time. For duplicate Telephone Bill, please contact
nearest Customer Service Center (CSC).
C) For any complaint relating to faulty Telephone or delayed / non receipt of Telephone bills please
dial Toll Free Number 0800 – 44544 for which call charges are borne by PTCL. While dialing
Toll Free Number please quote your local “18” complaint number for prompt rectification &
accountability. Service is available from 0830 – 1630 HRS on Monday – Thursday & Saturday.
On Friday 0830 – 1230 HRS.
