    Required Document Policy

    The following required information/documents must be included with all grant proposals. In each case, the documents should be from the applicant organization.* List of the organizations board of directors, officers and key staff Most recent audited financial statement, completed within 2 years; OR most recent IRS Form 990 including Schedule A , completed within 2 years (990 EZ or long-form format only; 990 ePostcard not accepted). o If an organization does not have a current audited financial statement or IRS 990 form, it must have a fiscal sponsor. o If an organization applies with a fiscal sponsor, a Fiscal Sponsor Agreement must also be included with the grant proposal.

    *If an organization is used as a fiscal sponsor, it must submit the documents noted above.
