Description of the Patient: Day 1 (January 29,2014) Awake, lying on bed in supine position, well groomed and hair

is properly tied, with patent IVF of D5NM 660 left, hooked at right metacarpal vein infusing @ 30 gtts/min. Complaints of hypogastric pain secondary to intake of contrast media for CT Scan, with watery stool but no complaints of pain upon defecating. Not in respiratory distress, skin is warm to touch, afebrile, T-36. Day 2 (January 30,2014) Received patient from Operating Room, Flat on bed, appears weak & drowsy, with intact dry dressing, not in distress, no complains of any pain, afebrile, T-36.8.
