India Visa Application Form: You must complete the India Visa Application Form. After the application
has been completed, you must print and sign the application.
It is very important that the following fields are filled in correctly, otherwise your application will not be
accepted as Travisa Outsourcing is not allowed to modify these fields on your behalf:
Indian Mission: USA-Houston. If you apply at a different city, you will need to fill out a new application.
Period of Visa: Duration of visa is selected in months, beginning on the date the visa is issued. For
example, requesting 60 months is requesting a 5 year visa. Please ensure that you request a Period of
Visa that will cover from the date the visa is issued to last day you plan to be in India. We strongly
recommend at least a 6 month duration.
No. of Entries: We strongly recommend multiple entry. Please ensure that you request the correct
number of entries to meet your travel needs to India.
Surname: This is your last name as on your passport.
Given Name: This is your first name and middle name (if applicable) as on your passport. Each name
should be separated by a space.
Passport No.: Passport number as on your passport.
Date of Expiry: Passport expiration date as on your passport (DD/MM/YYYY format).
The application should be printed on 2 separate sheets of paper.
Digital Image of Visa Photo and Signature: A digital image of the first page of your visa application
containing your visa photo and signature will be required at the time of completing the Travisa Order
Form.
Processing delays of 48 hours or more will result if you do not upload your visa photo and signature
when completing the Travisa Order Form.
Travisa Order Form: After you complete the India Visa Application, then complete the online Travisa
Order Form. This form allows Travisa Outsourcing to provide real time tracking of your application and
indicates your mode of payment and return shipping choices. You MUST include a copy of this form with
all other supporting documents.
Signature: The India Visa application has 2 places for signature. The following sections must be signed
correctly:

Page 1: Each applicant must sign COMPLETELY inside the box on Page 1 below their picture. Minors who
cannot write their name can use a thumbprint in black ink instead. If any part of the signature or
thumbprint is outside of the box, the application will be rejected.
Page 2: At the bottom of Page 2, the applicant must sign. If applicant is a minor, both parents or legal
guardians must sign.
Passport: Passport must have a minimum validity of six months from the date of application with at least
two blank visa pages. Last 2 amendment pages are not acceptable. Must be the actual passport not a
copy because your visa will be stamped in the actual passport.
Photograph: One latest 2x2 inch passport size photographs (black/white or color) depicting front pose
against white background on photographic paper is required. No glasses are allowed to be worn in the
photo. Using glue or paste, affix the photo to the India Visa Application. Do not staple the photo.
Proof of Address: Applicants must submit a photocopy with proof of residence/address that matches the
present address on the visa application exactly. Photocopy of state issued ID (Driver License or ID),
major utility bill (Water, Gas, Electric, Sewage) or a copy of a valid/current lease containing both the
landlord and tenant signatures must be submitted. Do not cut your drivers license copy down to a
smaller size. Please leave the copy on regular sized paper. If you don't submit a copy, we will make a
photocopy for you for $2.00.
The following items are not accepted: cell phone bill, credit card statement or bank statement. The
address cannot include a P.O. box.
For minors, a proof of address from both parents or a legal guardian may be submitted instead.
For College/University students living on campus in university housing/dormitories, a letter from your
institution on the institution's letterhead explaining the exact address that you presently reside in will
suffice.
Flight Itinerary: A computer generated flight itinerary from the airline or travel agency or copy of airline
or cruise tickets.
For Minors Only - Copy of Birth Certificate: For minors only (under 18), aphotocopy of the birth
certificate or any government issued document that shows parents name, date of birth and nationality.
Depending on your nationality at birth, these are the documents that are accepted.
US Born: Only birth certificates are accepted for applicants born in the US.
Indian Born: Those born in India can use their birth certificate, old Indian passport or school leaving
certificate.
All Others: Those not born in the US or India should submit a birth certificate or any government
document from the country of origin that has parents name, date of birth and nationality. Must be
translated into English and notarized.

Minor applicants do not have to be present provided that both parents have signed the application. One
parent may submit in person. You should not send the originals - we cannot be responsible for original
documents. If you don't submit a copy, we will make a photocopy for you for $2.00.
For Minors Only - Parents' Passport Copies: A copy of the vital page(s) containing the picture and
signature page of both parents signed passports. If applying in person, please note that the minor does
not need to be present. Only one parent will be permitted on the premises to submit the minors.

For Minors Only - Parents' Signatures Notarized: Visa applications for minors should contain both
parent's signatures which have been duly notarized with a notary stamp and seal. Both parents must be
present at signing the application before the notary. This applies to applications submitted By Mail only.
Admission Letter: A letter confirming admission from a recognized institution along with evidence of
financial arrangements for stay in India.
Medical or Paramedical Courses: In case of admission in medical or paramedical courses in India, a letter
of Approval or No objection certificate from the Ministry of Health, Government of India is required.
Engineering/Technical Graduate Courses: For admission to graduate or post-graduate courses in
engineering/technical institutions in India, a letter of approval or a No objection certificate from the
Ministry of Human Resources Development (Department of Education) is required.
Protected/Restricted Area Permit: Applicants planning to travel in the following areas in India will need
to apply for a special protected / restricted area permit. Arunachal Pradesh, Parts of Himachal Pradesh,
Parts of Jammu and Kashmir, Parts of Rajasthan, Sikkim and Parts of Uttarakhand. The permit fee is
$30.00 in addition to the standard consular fees. A completed Protected Area Permit Form is required:

take several weeks to process. Processing time cannot be guaranteed and tickets should not be
purchased until all visas are granted and in your possession.
Visa Status: If you are a non-US citizen, then you must provide a copy of one of the following: your
Permanent Resident Card (both sides), Employment authorization card or copy of page showing H1 / H4,
L1/L2, F1/F2, J1/J2 etc. Non-US passport holder parents who are applying for their kids must also
provide US visa status. Do not send the original documents. Travisa Outsourcing cannot be held
responsible for loss or damage to original documents. Non-US citizens applying for Business or
Employment visas must meet a minimum of 2 years living in the US on a long term visa or permanent
resident card. Otherwise, the applicant must apply in their country of permanent residence. (Non US
Citizens - Permanent Resident/Green Card Holders Only)
Short Term Visa Holders: Applicants who are not citizens/residents of the USA and hold a tourist or
other short-term visa for the USA should apply for Indian visas in the country of their permanent

residence. In case of emergency or special circumstance, an application could be made at the Indian
Embassy in Washington DC or Consulates General of India in the USA. In addition to the relevant visa fee
there is a reference fee of $20.00 for reference to be made to the country of their original residence.
Such cases have a minimum processing time of one to two weeks, and some cases may take longer.
(Non US Citizens - Short Term Visa Holders Only)
Permanent Address in Home Country: While completing the online visa application, your permanent
address in your home country is required under 'Permanent Address'. A relative's address may be used if
you no longer maintain an address in your home country.
Dual Citizenship: Applicants who have dual citizenship with the US and another country must apply on
their US passport, if living in the US. The exception is for applicants who hold dual citizenship with
Pakistan. Persons who have held Pakistani citizenship, and have also acquired US citizenship must apply
for a visa on their Pakistani passport, with the only exception being for those that can prove that they
have renounced their Pakistani citizenship with a copy of their renunciation certificate. Absence of
Pakistani passport or an expired Pakistani passport is not considered proof of renunciation of Pakistani

Long Term Visa Holders Registration:
Registration formalities for foreigners visiting India on Long Term Visa
List of documents required at the time of registration with the Foreigners Registration Officer.
(i) Photocopy of the passport and initial visa.
(ii) Four photographs of the applicant.
(iii) Details of residence in India.
(iv) Copy of the marriage certificate in case of those seeking extension of stay on grounds of being
married to an Indian national.
(v) Bona-fide certificate from the University/college/institution in case of student visa.
(vi) Accreditation certificate from the Press Information Bureau in case of Journalist visa.
(vii) Approval of the Department of Company Affairs in the case of board level appointees in Public
limited companies.
(viii) Two copies of the approval of Government of India in case of a joint venture or a collaboration.
(ix) Terms and conditions of appointments and copy of contract or agreements, in case of employment
visa.
(x) Undertaking (in the format given below) from the concerned Indian company on the following lines
in case of employment/business visa: UNDERTAKING

Processing Time: Persons of Indian Origin, US citizens who require a reference and all Non-US citizens

All Consular Fees and Processing Fees are NONREFUNDABLE, even if a visa is denied or if a visa is issued
for a shorter period of time or otherwise issued or returned at a time or on terms and conditions that
vary from those sought by the applicant.
Travisa Outsourcing is a service company only. Travisa Outsourcing does not issue visas or in any way
determine or control the issuance of, or terms and conditions of, visas all of which is determined and
controlled solely by the Consulates of the Embassy of India. Travisa Outsourcing is not and cannot be
responsible for any determinations, acts or omissions by the Consulates.

$0.00 - Travisa Outsourcing uses your Prepaid Airbill. If you choose this option, please provide a prepaid
airbill along with your application so your documents can be returned to you once your application has
been processed. Do not provide a hand written UPS or FedEx air bills. Airbills with credit card
information, handwritten airbills or blank airbills with information typed in cannot be used by our office.
Computer generated labels which can be printed from the shipping company's website are acceptable. A
copy of the return shipping label or prepaid envelope is also required. If you don't provide a copy, we
will make a photocopy for you for $2.00.
If you are applying from a US military base outside of the United States, please enclose a prepaid
shipping label to return your documents.

Include the 9 digit Application Id of all applications on the bottom right of the shipping envelope for
tracking purposes. We strongly recommend using a trackable method such as FedEx.
Please note that we recommend using FedEx or UPS before using registered mail due to the fact that
USPS tracking does not show up in the USPS website until up to 48 hours later.
