If the parents and authorized physician named cannot be reached at the time of an emergency and if immediate observation or treatment is urgent in the perception of school authorities, I authorize that my son/daughter be taken to the hospital for emergency medical treatment. I agree to reimburse the school for any medical costs that might be incurred by my son/daughter while on the trip.

I also understand that in the event that my son/daughter does not comply by the rules given to them or by the rules of the school, they may lose privileges on the trip and may be required to be chaperoned at all times. Consequences may be handled upon return to school or they may also be flown home at my expense and will not receive a refund of any kind. Major violations include but are not limited to:

TRAVEL &. PACKING TIPS fOR STUDENT GROUPS

UJJGGAGIE & PACKING

You are allowed one suitcase and one small carry-on bag. Performers are also allowed a garment bag for their uniform and an instrument case with any necessary equipment, All of these items are your responsibility through the duration of the trip. You will need to be sure that your items are placed in the correct loading area so it gets placed on the bus. Each bag and instrument case should be clearly marked with your name for easy identification. Remember, what you pack is what you have to carry! Also, please remember to bring the keys if you lock your luggage or instrument.

Be sure to pack your carry-on bag with a sweater, hairbrush, toothbrush, toothpaste, medications, eye care, and wallet. If your itinerary calls for your first destination to require a bathing suit, beach towel, sun block, or beach shoes, please place them in your carry-on too. You will not be allowed to get to the items under the bus until it is time to check into the hotel.

Please dress comfortably for you may be sitting, walking, or standing for long periods of time. You are also expected to dress appropriately which means no t-shirts with questionable wording, short-shorts, mini skirts, or half-shirts. The school's dress code will be in effect during this trip.

BUS INSTRUCTIONS

Be sure to check with the chaperones to find out the next departure time. Frequently, we may be on a tight schedule and it will be necessary to get everyone together without delay so don't be late! At all times, show courtesy and consideration for others traveling with you. All music players must be used with headphones and any uneaten food or food wrappers must be disposed of properly. Please keep the bus clean of trash by bringing it forward to the trash bags. Everyone is expected to sit down while the bus is moving and keep noise to a minimum so the drivers can concentrate on driving. You cannot use cologne, perfume, hairspray, or nail polish while on the bus. Do not bring glass containers or squirt guns with you on the bus. You can bring (school-approved) movies or games to help pass the time. You may want to consider bringing some things to help make the ride more comfortable such as a small pillow. No instrument playing on the bus!

HOTEL INSTRUCTIONS

Phones will only be available to use room to room. All long distance, calling card, and local phone calls must be made from a pay phone. A credit card is necessary to activate the room phone, movies, or games. PLEASE DO NOT LEAVE MONEY OR VALUABLES IN YOUR HOTEL ROOM OR ON THE BUS! You can put them in a safe, keep them with you, or give them to a chaperone. If something is extremely valuable, please leave it at home.

Please be courteous to other hotel guests by keeping the noise down and conducting yourself appropriately. At no time will you be allowed to enter the room of a member of the opposite sex or someone not with the group. During "lights out" time, you are expected to be in your assigned room with the lights off, please get ready for bed before this time. Security will be checking to be sure that your lights are off at the appointed time. Keep your door locked at all times and verify who is at the door before opening it. Also, be sure that to collect all of your belongings from the hotel during check out for once we [eave you might not be able to retrieve them.

Remember •••

Watch your sun exposure! Use a high SPF sun block if necessary. You do not want to spend your whole trip in agony from being sunburned. Tell your director or a chaperone if you are having a problem or fee! sick, they are there to help you.

Plan on creating a daily budget so you don't spend all of your money. Plan to spend $8.00 - $10.00 for each meal that is not provided with the trip. Remember that you will need money for the trip home
