Introduction
CIESTheFoodBusinessForumisthe onlyindependentglobal food business network. Itbringstogetherthe CEOs andseniormanagementofaround400retailerandmanufacturermembersofallsizes,across150countries. It has a mandate from itsmemberstodevelopcommonpositionsonkeystrategicandpracticalissuesaffectingthefoodbusiness. The Global Food Safety Initiative (GFSI), coordinated by CIES The Food Business Forum, was launched in May 2000. The GFSI Foundation Board, a retailerdriven group, with manufacturer advisory members, provides the strategicdirectionandoverseesthedailymanagementoftheGlobalFoodSafetyInitiative. TheGFSIMissionistoworkoncontinuousimprovementinfoodsafetymanagementsystemstoensureconfidence inthedeliveryoffoodtoconsumers. TheGFSIObjectivesareto: Maintain a benchmarking process for food safety management schemes to work towards convergence betweenfoodsafetystandards,asoutlinesinthisGuidanceDocument. ImprovecostefficiencythroughoutthefoodsupplychainthroughthecommonacceptancefoGFSIrecognised standardsbyretailersaroundtheworld. Provide a unique international stakeholder platform for networking, knowledge exchange and sharing of best foodsafetypracticeandinformation. The GFSI Foundation Board also provides governance to the GFSI Technical Committee, an international multi stakeholder group of over 50 food safety experts. The GFSI Technical Committee works on specific selected projects throughout the year, approved by the GFSI Foundation Board in order to fulfil the GFSI Mission; these have included projects such as auditor competence, food defence and technical issues relating to emerging and developingmarkets. In2008theGFSIFoundationBoardformedtheGFSIAccreditationTaskForcewhosepurposeisto: agreeoncommonareasofconcerninrelationtotheaccreditationprocess,byundertakingareviewofcurrent documentsfromGFSIbenchmarkedschemesregardingtherelationshipbetweenCertificationBodiesand AccreditationBodies,inordertoidentifyanddefinecommonrequirementsandindividualrequirements gainandcollateinformationfromGFSIbenchmarkedschemeownersonanyoperationalissuesregardingthe accreditationprocessandseektheiragreementtoactontheirbehalfregardingthedevelopmentofcommon requirements reviewtheagreedcommonrequirementsidentifiedagainstISO17011andputforwardareaswhereISO17011 canbemorespecificinrelationtotherequirementsofGFSIrecognisedschemes developandagreeaRequirementsdocumentdefiningcommonrequirementsforAccreditationBodiesin relationtoISO17011andGFSIrecognisedschemes usetheagreedRequirementsdocumentasthebasisfordiscussionwithInternationalAccreditationForum andEuropeanAccreditation,withaviewtogainingrecognitionofthedocumentbytheseorganisations agreeimplementationcriteriafortheproposedRequirementsdocumentsallowingGFSItoactonthebehalfof theAccreditationTaskForceandGFSIrecognisedstandardsowners undertakethereviewoftheprogressofthedevelopmentandimplementationoftheRequirementsdocuments andadvisetheGFSIFoundationBoardaccordingly.

ThePurposeoftheGFSIRequirementsontheApplicationofISO/IEC17011
ISO/IEC17011:2004isanInternationalStandardwhichsetsoutcriteriaforconformityassessmentfor AccreditationBodiesaccreditingconformityassessmentbodies.Therearenospecificrequirementsinrelationto thoseAccreditationBodiescarryingoutassessmentofconformityassessmentbodiesworkingwithfoodsafety schemes. Since2000theGFSIhasworkedcloselywithanumberofmajorstakeholdersinthefoodindustryinanattemptto harmoniseprocessesrelatedtofoodsafetyschemesandhasbenchmarkedandrecognisedfourpostfarmgate schemes. Overaperiodoftime,eachoftheseschemeshasdevelopedtheirownsetofcriteriainrelationtothe accreditationprocesswhichhasleadtoadiversityofrequirements;thisinturnhasleadtocomplexityofprocess,, increasedcostandalackofclarity. TheGFSIAccreditationTaskForcewasformedinFebruary2008withanobjectivetofacilitatedialoguebetween GFSIrecognisedschemeownersandotherinterestedpartiesinordertoharmonisetheirapproachandminimise theeffectsthatthediversificationofrequirementshascaused. ThisdocumentistheculminationofworkundertakenbyGFSIandthefourGFSIrecognisedschemeownersto specifyharmonisedrequirementsinrelationtotheaccreditationofconformityassessmentbodiescarryingout productcertificationtotheGFSIrecognisedschemes. ThedocumenthasbeenentitledRequirementstoreflectthatthisdocumentisnotregardedasguidancenotes andwillbethesubjectoffurtherworkwiththeGFSIandtheGFSIrecognisedschemeownerstoensure compliancewiththespecifiedrequirementsbythoseAccreditationBodiesworkingwithGFSIrecognisedschemes. . Thetermshallisusedinthedocumenttoindicatethoseprovisionswhichareregardedasbeingmandatory. Therearerequirementswithinthedocumentwhicharespecificwithrespecttoparametersofassessment;these shallberegardedasminimumrequirementsanddonotprecludeAccreditationBodiescarryingoutincreased activityintheeventthattheywouldwishto,ordeemitappropriatetodoso. TheGFSIwishestocontinueworkcloselyIAFanditsmembersinaspiritofcooperationtoensurethe implementationoftheserequirements,whichwillprovideenhancedconfidenceregardingtheaccreditationof conformityassessmentbodiesworkingwithinthefoodindustry.

4.4Confidentiality GFSIRequirementstoclause4.4 The Accreditation Body shall share information regarding scheme specific issues with the appropriate representative of each GFSI recognised scheme owner. This information shall be shared as soon as practicable and the accreditation body shall be responsible for the followup of the corrective actions in thecaseofcomplaintsandmajornonconformities. Thislistofinformationtobeshared,whichisnotexhaustivebutshallinclude: accreditationstatus,(suspension,withdrawal,scopereductionorexpiryofaccreditation), complaintswhichcouldjeopardisetheaccreditationstatus, major nonconformities (a major nonconformity is defined as a nonconformity which has the potentialtoimpactonthevalidityofacertificateorofcertificatesissue,. annual activity summary which shall be made available to the GFSI recognised scheme owners indicatingasaminimum: Generalassessmentdetails Head office assessment: date, Location, critical location assessments, numbers of site and auditorfilesreviewed,AccreditationBodysassessorsnames,duration,generalconclusions Witness assessment: location, date, field, Accreditation Body assessor, duration, general conclusions CABnameandconclusionofreportsAccreditationStatus TheannualactivitysummaryshallbeinastandardisedformatasagreedbyGFSI(seeappendix). There shall be an agreement regarding confidentiality between the AB and the GFSI recognised standards owners. This agreement shall specify that the information shared shall be made known to all GFSI recognised scheme owners, but shall remain confidential from any other party. There shall also be anagreementbetweentheindividualCABsandtheGFSIrecognisedschemeowners. 4.5Liabilityandfinancing Noadditionalrequirements 4.6Accreditationactivity GFSIRequirementstoclause4.6.1 Normative documents shall include all documentation relevant to GFSI recognised scheme, and shall be issuedbyaGFSIrecognisedschemeowner. 5Management Noadditionalrequirements 5.1General Noadditionalrequirements 5.2Managementsystem GFSIRequirementstoclause5.2.1 With respect to effective communication, communication channels with interested parties shall be in placeandincludeGFSIandGFSIrecognisedschemeowners. 5.3Documentcontrol Noadditionalrequirements 5.4Records GFSIRequirementstoclause5.4.2 Recordsshallbemaintainedandaccessibleforaperiodofaminimumof5years. 5.5Nonconformitiesandcorrectiveactions Noadditionalrequirements 5.6Preventativeactions Noadditionalrequirements 5.7Internalaudits GFSIRequirementstoclause5.7.2 TheinternalauditshallincludeaccreditationactivitiesinrelationtoGFSIrecognisedschemes. 5.8Managementreviews Noadditionalrequirements 5.9Complaints GFSIRequirementstoclause5.9 All complaints associated with any GFSI Scheme shall be investigated and dealt with, as far as possible within 30 days of receipt. If any complaint puts a GFSI recognised scheme certificates/approvals, at risk, then the GFSI scheme owner shall be informed immediately and appropriate action taken by the AccreditationBody. 6HumanResources Noadditionalrequirements 6.1Personnelassociatedwiththeaccreditationbody Noadditionalrequirements 6.2Personnelinvolvedintheaccreditationprocess GFSIRequirementstoclause6.2.1 All AB personnel engaged in GFSI recognised scheme accreditation activity shall have sufficient knowledge of each relevant GFSI recognised scheme, related normative documents and the food industry. The AB shall ensure the assessment team undertaking witness activities includes personnel who have completed training in the relevant GFSI scheme, successfully completed a HACCP training course and haveanindividualwithaminimumoftwoyearsexperienceinthefoodindustrysector. The assessment team carrying out office assessments shall include personnel who have a specific knowledgeoftherelevantGFSIschemesbeingassessedandtherelatednormativedocuments. TrainingintheGFSIrecognisedschemeshallberecognisedbyrespectiveschemeowners. 7Accreditationprocess Noadditionalrequirements 7.1Accreditationcriteriaandinformation Noadditionalrequirements 7.2Applicationforaccreditation GFSIRequirementstoclause7.2.1 The Certification Body shall demonstrate that they have made appropriate contact with the scheme ownerduringtheapplicationprocedure. GFSIRequirementstoclause7.2.3 The AB shall inform a GFSI scheme owner when an application is made to accredit against a GFSI recognisedscheme. 7.3Resourcereview Noadditionalrequirements 7.4 Subcontractingtheassessment GFSIRequirementstoclause7.4.1 If the AB subcontracts an assessment, the assessment shall be carried out in accordance with this document and the individual GFSI scheme owners requirements. The sub contracted AB shall be a signatory to the IAF MLA for Product Certification and the assessor shall fulfil the competence requirements outlined in this document. Where additional formal agreements exist, e.g. IAF GD 3 Cross Frontier Policy. There shall be compliance with these agreements. The AB using subcontractors shall reviewthecompliancewiththeserequirements. 7.5Preparingforassessment GFSIRequirementstoclause7.5.8 During the surveillance and reassessments, the following documentation shall be sampled and assessed asaminimum: 1.Sampleofatleast10%or2auditorfiles,whicheverisgreater,foreachGFSIscheme 2.Sampleofatleast2sitefiles,foreachGFSIschemeor2%deliveredaudits,whicheverisgreater,for eachGFSIscheme 3.Samplingofsitefilesshallberiskbasedandshouldbedoneacrosscountry(region),productcategory andassessor 4.Thesitefileassessmentsshallincludeareviewof: ContractsbetweentheCABandtheclient Confirmationofauditscopeandduration Auditorsoriginalnotesfromauditandevidenceofcompliance Documentaryevidencesuppliedbytheclientfornonconformitiesidentified Finalauditreport CertificationDecisionandCertificateConformity Samplinglevels shallbeincreasedwhereevidence ofissuesareidentifiedduringtheassessmentinorder tosatisfywhetherthecertificationbodyismeetingaccreditationrequirements. Where objective evidence or complaints are identified between planned surveillance and reassessments, accreditationbodiesshallcarryoutadditionalassessments. GFSIRequirementstoclause7.5.10 TheABshallensurethattheGFSIstandardownerdocumentationisincluded.TheABshallrequest, throughdirectcommunicationwiththeGFSIstandardowner,anyfeedbackorrelevantinformation concerningtheCABinadvanceoftheassessment. The AB shall take into account during the assessment, any feedback or relevant information concerning the CAB sentbya GFSI recognised scheme owner. The AB must verify during the assessment, that these complaintshavebeendealtwitheffectivelyandanycorrectiveactionsputinplace..* *AppropriatecriteriadocumentsreferredtoinISO/IEC17011shallincludethecertificationbodycriteria establishedbytheGFSIrecognisedschemeowner. 7.6Documentandrecordreview GFSIRequirementstoclause7.6.1 TheGFSIrequirementsspecifiedinrelationtoclauses7.7.2shallapplyinadditiontothatrequiredunder clauses7.2.1and7.2.2. 7.7Onsiteassessment GFSIRequirementsforclause7.7.2 DuringtheinitialassessmentagainstthescopeofanyGFSIrecognisedscheme(ie.Anextensiontoscope, cf7.12),thefollowingdocumentationshallbesampledandassessed: 1.Sampleofatleast1auditorfilesfortheGFSIscheme 2.Sampleofatleast1sitefilesfortheGFSIscheme 3.Samplingofsitefilesshouldbedoneacrosscountry(region),productcategoryandassessor 4.Thesitefileassessmentshallincludeareviewof: ContractsbetweenCABandclient Confirmationofauditscopeandduration Auditor'soriginalnotesfromauditandevidenceofcompliance Documentaryevidencesuppliedbytheclientfornonconformitiesidentified Finalauditreport CertificationDecisionandCertificateConformity 5. Evaluation and verification of the effectiveness of the CABs processes for assessment of auditor competence GFSIRequirementstoclause7.7.3 AtleastoneassessmentoftheCABshallbewitnessedforinitialaccreditationforeachGFSIrecognised scheme.IntheeventthattheCABwishestoincreasetheirscopewithrespecttoproductcategorythis levelofsamplingshallbeincreased. WithCABswithmorethan20auditorsthenwitnessassessmentsshallbecarriedoutatarateofatleast 1witnessassessmentper20auditorsperyear,auditingagainstanyoftheGFSIrecognisedschemes. WhereinternationalCABhavegeographicalspreadthenthe%ofwitnessauditswillbeproportionalto thegeographicspreade.g.30%overseasthen1in3witnessedauditswillbeperformedoverseas.The witnessassessmentprogrammeshallbedesignedtoensuredifferentauditorsanddifferentproduct categoriesareassessedatsubsequentwitnessedassessments. GFSIRequirementstoclause7.9.5 TheaccreditationcertificateorscheduleshallincludethefullnameandversionoftheGFSIrecognised scheme,andtheGFsIrecognisedschemesdesignatedproductcategoriesforwhichtheyareapproved. 7.10Appeals Noadditionalrequirements 7.11Reassessmentandsurveillance GFSIRequirementstoclause7.11.3 Headofficeshallhaveanassessmentonanannualbasis. AwitnessassessmentofaCABshallbeconductedbytheABatleast1peryear foreachCAB.All GFSIrecognisedschemeswillbewitnessedina4yearcycle. WithCABswithmorethan20auditorsthenwitnessassessmentsshallbecarriedoutatarateof at least 1 witness assessment per 20 auditors per year, auditing against any of the GFSI recognised schemes. Where international CAB have geographical spread then the % of witness audits will be proportional to the geographic spread e.g. 30% overseas then 1 in 3 witnessed audits will be performed overseas. The witness assessment programme shall be designed to ensuredifferentauditorsanddifferentproductcategoriesareassessedatsubsequentwitnessed assessments. Reassessmentshallbeundertakenatleasteveryfouryears. GFSIRequirementstoclause7.11.5 The AB shall inform the GFSI scheme owner about any major nonconformities.( Ref clause 4.4; a major nonconformity is defined as a nonconformity which has the potential to impact on the validity of a certificateorofcertificatesissue.) GFSIRequirementstoclause7.11.6 TheABshallinformtheGFSIschemeownerifaccreditationissuspended,withdrawnorexpiredorscope isreduced. GFSIRequirementstoclause7.11.7 When informed of identification of issues in a particular market, which could bring the credibility of the certification process into disrepute, the AB shall take any appropriate action including conduct of an extraordinaryassessmentifnecessary. 7.12Extendingaccreditation GFSIRequirementtoclause7.12 AfullinitialassessmentshallbeundertakenforeveryGFSIrecognisedscheme,uponanapplicationto extendascopeofISOGuide65toincludeanewGFSIrecognisedscheme. IntheeventthattheCABwishestoincreasetheirscopeofactivitywithrespecttoproductcategoryas definedbyGFSIrecognisedschemeowners,theaccreditationbodyshallasaminimumundertakean adequatecheckofdocumentaryevidencetoensuretherequirementsoftheGFSIrecognisedstandards areinplace. 7.13Suspending,withdrawingorreducingaccreditation GFSIRequirementstoclause7.13.2 WhenGFSIrecognisedschemeownersrequirementsarenotmet,theGFSIschemeownerwillinform theAB.TheABshallundertakeaninvestigationoftheissuesraisedbytheGFSIrecognisedscheme ownerandshalltakeappropriateactione.g.tosuspendorwithdrawaccreditation. GFSIRequirementstoclause7.13.3 WhenGFSIrecognisedschemeownersrequirementsarenotmet,theGFSIschemeownerwillinform theAB,TheABshallundertakeaninvestigationoftheissuesraisedbytheGFSIrecognisedscheme owner,whoshalltakeappropriateactioni.ethedecisiontoreducethescopeofaccreditation. 7.14RecordsofCABs Noadditionalrequirements 7.15Proficiencytestingandothercomparisonsforlaboratories Noadditionalrequirements DateofIssueFebruary2009 NotforPublication Page13

8ResponsibilitiesoftheaccreditationbodyandtheCAB Noadditionalrequirements 8.1ObligationsoftheCAB GFSIRequirementstoclause8.1.2(f) IntheeventthatGFSIrecognisedschemeownerssuspendsorwithdrawsrecognitionforaCAB,or reducesscope,inadditiontothenotificationbytheCAB,theschemeownershallalsoinformtheAB. 8.2Obligationsoftheaccreditationbody GFSIRequirementstoclause8.2.3 TheABshallprovidethisinformation,whereapplicable,toGFSIrecognisedschemeowners. TheABshallinformtheCABsthattheyaresignatoriestotheGFSIrequirementstoISO/IEC17011. GFSIRequirementstoclause8.2.4 IntheeventofchangesorrevisionstoaGFSIrecognisedstandard,theABshallcollaborateand communicatewiththeschemeownertoidentifytheappropriatereviewprocessoftheCAB,andto ensurethattheCABisawareofandmeetstheadditional/changedrequirementsinrelationto accreditationrequirements. 8.3Referencetoaccreditationanduseofsymbols GFSIRequirementstoclause8.3.2 TheABshallverifythat8.3.2,points(af),areappliedinconnectionwiththeusageoftheGFSI recognisedschemes. GFSIRequirementstoclause8.3.3 TheABshallverifythat8.3.2points(af),areappliedinconnectionwiththeusageoftheGFSI recognisedschemesandappropriateactiontakenintheeventofincorrectormisleadingclaimsin relationtotheuseofsuchinformationandaccreditationstatus.
