Consent Form Small Business Survey

We are a small group of Louisiana State University students conducting research to observe the effects of social media on small businesses. This research is being conducted on behalf of our Public Relations Research course, MC 3020, offered by the Manship School of Mass Communication at LSU.

We intend to gather enough information to help small businesses better their company through awareness, brand loyalty and customer satisfaction. Our sample of participants will consist of consumers in the Baton Rouge area who have an online presence and another set of participants who are small business owners.

All participants have the right to withdraw from the survey at any time. Data that we collect is confidential in every way. This information will be used for research purposes only. Personal information will not be known to the researchers and will not be disclosed. There is no risk in participating in our survey.
