Counseling of Biological Parents Case Study Report prepared by licensed social worker should show proof that the support services required were given for the following purposes: 1. to exhaust all measures to strengthen family ties and keep the child Deed of Voluntary Commitment (IRR)2. to ensure that their decision to relinquish their child for or the Affidavit of adoption is not caused by Consent (SC Rules stress anxiety or pressure of Adoption) 3. to ascertain that such decision does not result in and improper financial gain for Written Consent of those involved in it Biological Parents

Written Consent of either: 1. The Legal Guardian 2. Proper Government Instrumentality which has legal custody of the child
