Purpose: To compare the diagnostic accuracy of a direct digital radiography system (DDR-CMOS) type with four types of image

filters and radiographic digital subtraction radiography (DSR) and recorded a priori and a posteriori in the diagnosis of defects produced artificially the dental crown. Methods: A total of 20 healthy premolar teeth extracted for orthodontic indications and severe periodontal disease. The teeth, arranged in pairs, 10 blocks were mounted on silicone addition. Each block was imaged under conditions standard techniques in intraoral radiographic apparatus with mechanical calibration previously investigated the technical factors and fixed exposure of 70 kVp, 8 mA for 0.4 seconds. To capture the image sensor used the Kodak RVG 6100 and is archived in TIFF format. The record was subsequently performed with the program Regeemy 0.2.43 and the DSR software Image Tool 3.0. The images were obtained with filters using the software Kodak Dental Imaging 6.1. The total number of images was 360, and these were evaluated by three expert examiners in dental radiology. Results: The sensitivity and specificity of the area under the ROC curve (Az) were higher in SRD images in diameter drills , and 1 (0.896, 0.979 and 1.000 respectively), showing a statistically significant difference compared to other methods. The highest values were found in drill a larger diameter and smaller for drill , and the lowest was in the negative filter (Az = 0.631). Clinical Significance: To evaluate which imaging method is more effective in the diagnosis of defects produced artificially simulating cavities and present a free program to download DSR (a posteriori) can be used in day-to-day clinical management of dental surgeons to diagnose tooth decay or enamel defects Keywords: Radiographic Image Enhancement .
