CHENNAI: This summer, 16-year-old Aakash isn't going to the Gulf to visit his parents as he usually does. Instead, the student, who stays in a hostel in Andhra Pradesh, drove to Chennai with his father to go under the knife. He is undergoing gynecomastia or surgery for male breast reduction. Hundreds of teenagers like Aakash studying or just completing high school make appointments with plastic surgeons during the summer break. "The number of school children undergoing plastic surgery is a fraction of the adults," says Dr Karthik Ramaswamy, chief surgeon at Chennai Plastic Surgery. But the numbers are high enough for plastic surgeons to consider April, May and June the peak season for cosmetic surgery.

Doctors see a 30% to 40% increase in the number of surgeries done on children during this period. "During April and May I do around 40 to 50 surgeries compared to 20 or 30 surgeries in September-October," says Dr R Murugesan from Apollo Hospitals and former president of Indian Association of Aesthetic Plastic Surgeons.

Plastic surgeons say teenagers usually come in for breast reduction and reconstruction, nose correction or rhinoplasty, and liposuction. "They opt for the extreme measure only if it is something like breast reduction, which can be traumatising for teenage boys," says Dr Ramaswamy. Dr Murugesan concurs and adds that parents fund only surgeries they consider a necessity. In Aakash's case, his father spotted the problem and contacted Chennai Plastic Surgery. "He sent us pictures and his son's medical details, and we showed how we could help," says Dr Ramaswamy. For boys like Aakash who live in hostels, having male breasts is more traumatizing than it is for day scholars. "They have to undress in front of other boys. So they shower last, wait for everyone to leave and do not participate in sports," he says. Cosmetic surgery is no longer an option only for the elite, though it is expensive. Parents from lower middle-class backgrounds are willing to scrape together money to make their children feel better. The 19-year-old daughter of a village nurse from Krishnagiri, who has just completed class 12, turned up at hospital with tears in her eyes. She said her breasts were too large and sagging. She planned to join a nursing college but suffered an inferiority complex because of her chest. "We told them the reconstruction would cost Rs 1.3 lakh and would leave a scar. They agreed and have paid Rs 40,000 promising to pay the rest in instalments," says Dr Ramaswamy. They prefer summer because they get time to recuperate. Breast reduction requires two days to heal, and five days later the stitches are removed. Rhinoplasty takes longer. "It takes two to three weeks for the initial swelling to go down and nearly six months for the entire swelling to go. There is no trace of the surgery," says plastic surgeon Dr Sivakumar from Right Hospital in Kilpauk.

As long as these teenagers use certified and professional plastic surgeons, everything's fine. I have nothing against them. Last year, when I had my liposuction procedure, I was also in the hands of a good and skilled surgeon, namely, Dr. Jerome Edelstein. Thanks to this great doctor, I achieved excellent results. With this cosmetic surgery, I really improved the overall contour of my body!

Dr Ramaswami like most other doctors and private hospitals is minting money at the cost of patients. No one controls the bills given by these hospitals. Government must lay down the cost of various surgeries and treatments that are well defined and finite in medical terms.

Indian parents are not very keen on encouraging their kids to indulge in sports or other calorie shedding activities. And this is more prevalent in rich families. Most of them have protruding belly or breast even in their early teenage years. Raised and fed at home to the best, this is inevitable if this trend continues. Give your kid a break, let him sustain an injury while he plays, no big deal. Problem solved!

The whole country is corrupted to the core - one crore for MBBS, one crore for PG , appollo's name appeared in 2G, backhanders for referring drs, corrupted decision making - I have first hand knowledge of hospitals leaning on the radiologists - to reports in such a way that unwanted surgery is performed!!! Who and how the system is going to change even god doesn't know. the poor patient continues to suffer. I had to leave the country to pracitce what I thought was right. Medical practice has become a market - and obviously good surgeons like lawyers can demand the fee which the market can afford!! UK's NHS is a good compromise - though not perfect it definitely meets all the criteria of good medical practice. The Indian corrupted private lobbies will never allow a system like the NHS to be created in India.

insecurity and the need to look polished in society is the main reason for this trend. It cant be helped and regarding Aakash, hitting the gym would have been a better option but I understand your situation. The girl needn't have worried because guy prefer bigger breasts but I guess there were medical reasons behind her opting for reconstruction surgery.
