participate in the program. In the event that I am unavailable for the purpose
of providing parental consent, I authorize the camp director and the camp
trainer to act for me according to his / her best judgment in any emergency
requiring medical attention. I agree to not hold South Western School
District or any member of the camp staff liable for any injuries should an
accident occur. I understand that I am financially responsible for any
medical bills incurred by my child while at camp.
