Lafaz Taliq:
Saya (nama suami) dengan ini bertaliq bahawa
jika:
1) Apabila saya tidak bermuasyarah dengan isteri saya (nama isteri)
. sama ada saya meninggalkan dia dengan sengaja atau
paksaan atau dia meninggalkan saya dengan paksaan selama empat
bulan qamariah berturut-turut atau lebih; atau
2) Saya atau wakil saya tidak memberikan nafkah kepadanya selama empat
bulan qamariah berturut-turut atau lebih sedangkan dia taat kepada saya;
atau
3) Saya menyebabkan mudharat terhadap tubuh badannya
kehormatannya atau penganiayaan atau sebarang dharar syarie

atau

dan kemudian dia mengadu kepada Mahkamah Syariah dan jika sabit
aduannya di sisi Mahkamah Syariah maka pada ketika itu jatuhlah talaq saya ke
atas isteri saya.
B. INGGERIS
Declaration of Taliq:
I (name of husband) hereby make a declaration of
taliq that if:
1) Whenever I did not muasyarah with my wife (name of wife).
whether voluntarily or with force or she leaves me under my force for a
period of four months qamariah continuously or more; or
2) I or my representative do not give her maintenance for a period of four
months qamariah continuously or more whereas she is obedient to me; or
3) I cause hurt to her person or her honour or cause abuse or any dharar
syarie
and then she makes a complaint to the Syariah Court and if it is found by the
Syariah Court to be true, then at that time I proclaimed talaq on my wife.
