REPORTING FOR ROOT CAUSE ANALYSIS

RCA program champions and facilitators often ask us for RCA report and chart templates or forms.
Because every organization, situation and problem is unique, our clients are most successful at truly
solving and preventing problems when they view root cause analysis as a knowledge-based process,
rather than a software- or template-based process. Further, valuable solutions come from people through
their ideas and process knowledge, and through the new information uncovered by the questions that are
generated and answered when creating the cause and effect chart.

DURATION 1 DAYS

COURSE OUTLINE

Module 1: Report Writing: A discussion of the report format, structure, and content.
Also, the possible problems with report writing are discussed.
Laboratory/exercise: Using the same Condition Report and the appropriate format,
select and edit a key section of the report. Be prepared to present the changes, explain
why you chose to edit that section, and the significance of the changes that you made.
Discussion of Laboratory work.
Module 2: Corrective Action and Follow-up: Discussion of the activities associated with
the completion of the investigation, including an effectiveness review plan and
monitoring of the implementation of the corrective actions.
Laboratory/exercise: Identify key variables associated with corrective action
implementation, e.g., key players, timing, material included, etc. Discussion of
laboratory work.
