Submit requirements via Benefits Dropbox at the reception area.

Please write a note that the purpose of

submission is for Pag-ibig loan application.

a. Accomplished Pag-ibig Loan/HDMF Application Form

b. Photocopy of Company ID

c. Photocopy of any Government ID

d. Certified True Copy of latest one month payslip
i. Log a ticket via myRequests under ask a payroll question ( no need to wait for the SR to
be resolved, just get the SR number )
ii. Print your latest 1 month payslip via via MEISS
iii. Send the document to payroll team, UG 02.E CG2 indicating the SR number (Important
Reminder: Do not forget to write the SR #)
iv. Indicate your exact location (e.g. 4.22K McKinley A) for the signed payslip to be returned
to you.

***Note: kindly wait for your certified pay slip to be returned to you before routing the complete
documents to the benefits team.

e. Most Recent Employees Statement of Accumulated Value (ESAV) from Pag-ibig. This is intended for
employees with previous employer/s. Should reflect that the latest posting date of contributions should be
within 1 to 2 mos. and under Accenture Inc.

f. Accomplished Pagibig Citi Prepaid Cardholder form for employees without Pag-ibig Citi Card
g. Photocopy of Citi Prepaid Card Back to back for employees with existing Pagibig Citi Prepaid Card Account.
Take note of the following details that you need to include in the form:
Signature over printed name in the promissory note portion and the application agreement portion of the
form
Thumb marks
Signature over printed name of 2 witnesses (colleagues/relatives)
List of previous employer/s (if any)

NOTE: You may submit the form to PAG-IBIG Makati 1 Branch at Justine Bldg, Buendia Ave., Makati City for
independent filing (after the form has been signed by HR Admin); just attach a note that you are requesting for
signatory of your loan form for independent filing.

For Cebu employees, processing of PAG-IBIG loan is centralized through Accenture Manila Office.
