Simple Loan Agreement

Know all men by these presents:

This specific loan agreement is made and entered into by and between:
NANDIE JOY G. ALVAREZ, of legal age and a resident of Jaro, Iloilo City, Philippines,
hereafter referred to as the CREDITOR;
-and-
MA. JUMA M. DOMINGO, of legal age, married and a resident of Brgy. Tubang,
Maasin, Iloilo, Philippines, hereafter referred to as the DEBTOR.
WITNESSETH:
That the CREDITOR is a co-employee of the DEBTOR in the Iloilo Provincial
Government and willing to lend money to the DEBTOR for her personal needs;
That the DEBTOR asked and agreed to borrow money from the CREDITOR;
That the DEBTOR is currently working as jobhire being paid by the Iloilo Provincial
Government and assigned as dental aide at Maasin Rural Health Unit, Roxas Street, Maasin,
Iloilo;
NOW THEREFORE, in view of the above, the CREDITOR hereby agree to lend to the
DEBTOR and the latter has agreed to borrow money from the CREDITOR, subject to the
following terms and conditions:
The CREDITOR will lend the DEBTOR a total amount of ___________________.
The DEBTOR has to return the total amount within a stipulated period agreed by the
parties at an interest rate of 5%.
In case there is a delay in paying the installment by the due date of a specific month, the
DEBTOR will be charged a late payment fee.
In case if the DEBTOR is unable to pay the installment for three consecutive due dates,
he will be considered a defaulter and the CREDITOR has all the right to file a suit against
the DEBTOR for Summary Proceedings of SMALL CLAIMS.
That this agreement shall be binding upon parties.
IN WITNESS WHEREOF, the parties have herunto set their hand this day of September
25, 2013, in the City of Iloilo, Philippines.
