    I've done it with an DS-1 M5223AL / TA7136P 1 / NC 2 / NC 3 / 2 4 / 4 5 / NC 6 / 3 I had to do it off board to fit. You may have to remove a 1n4148 diode depending on the version.

    1/ NC
    2/ NC
    3/ 2
    4/ 4
    5/ NC
    6/ 3
    7/ 6 to 150pf cap to pin 1 of TA7136P
    8/ 7 to 27k resistor to pin 5 TA7136P
    I had to do it off board to fit. I use a 7 by 4 hole piece of strip board.
    You may have to remove a 1n4148 diode depending on the version.
    I'm surprised Monte Allums hasn't made a op-amp adapter for this.
