A 41 Year Old Man

with Ulcerative Colitis

INTRODUCTION
Ulcerative collitis is the most common form of inflammatory bowel disease
worlwide. The risk factors for inflammatory bowel diseases are westernized
environment and lifestyle, smoking, diets high in sugar and fat, medication

use, stress, high socioeconomic status and appendectomy.
CASE REPORT
41 year old man, admitted to the RS DR Kariadi hospital at Dec 20, 2013 with
chief complaint : bloody stool
History : patient reported black reddish stool, no abdominal pain, nausea
but not vomiting. He had recurrent diarrhoea and bloody stool since 2008.

Dietary history : reduced intake (1900 kcal to 1300 kcal), low consumption of fruits and vegetables, frequent consumption of fried and fatty food,
drinks alcohol and cigarette smoking were recorded during interview. He
preferred eating porridge. He avoided eating meat and only consumed
tofu, tempeh and egg for protein source and occasionally had milk. The
body weight is decreasing, from 58 kg to 48 kg.
Disease history : Multiple Polip Duodenum (2011), Chronic Gastritis Atrophy Non Metaplasia (2013)

Medical diagnosis : Lower gastrointestinal bleeding, ass : diverticel; malignancy; anemia microcytic hypochromic, ass : chronic disease, bleeding
Nutrition diagnosis : Nutrition status : underweight, anemia; metabolic status : hypermetabolic; gastrointestinal status : melena.

MONITORING & EVALUATION


AFTER 2 MONTHS

Discussion :

FOOD INTAKE MONITORING

The most probable causes of UC in this patient are the history of


low fiber intake, high saturated fat diet and cigarette smoking habit. When the small amount of lower GI bleeding persist, the patient
can be given oligomeric formula, then gradually increases to polymeric and regular diet depends on clinical development.
The omega 3 fatty acid and zinc have anti inflammatory effect, so
that the supplementations are useful.

Conclusion :
Nutrition intervention is the key role of ulcerative colitis
improvement.
Supplementation of anti inflammatory agents maybe useful.
