This is in connection with my failure to log out last November 29,2013 Friday after my
RLE duty for Level 2 a s a replacement for an absent Clinical instructor for the said date. This
was my first duty after a long time which i think resulted to my not being accustomed with the
routine and really an honest memory lapse. Attached with my letter is the time in/out sheet of the
