Patient revealed signs of delusions regarding of his parents death, such as: making
unrealistic stories. He believes that all he was saying was true.
Patient feels lonely as he further stated Wala nang pakialam yung mga kamag-
anak ko saakin at patay na rin ang mga magulang ko. hen asked if how he copes with
his loneliness, he stated !"kay naman may nakakausap naman ako ditto sa ward.#
H. Cognitive
Patient is well oriented to time and place. He knows how long he has been staying
in the hospital already. He is aware that he is currently admitted at the ward. $emote
memory is still intact as he can recall past events in his life, including those significant
ones.
Impression: Patient can be classified under %&is ' since he is diagnosed with
(chi)ophrenia and there is depression reported.
