With reference to your memo I hereby submit to you following facts
On 05/02/2014 .I was in the morning duty and at 1300hrs Urdu bulletin,
echo was observed after some time in the output monitoring for few second, performing fold back level I reduced the Fold back level first when it didnt help. I noticed that FS/3 was jacked in very low level which was jacked out completely and audio become ok (i.e echo) this might have occurred due to hand movement while operating the faders The operational mistakes however regretted and I will be most careful in future.
