Making Your Group Benefit
Elections for the 2014-15 Plan Year
For technical assistance in making your 2014-15 Elections online, call Big Oak Advisors at
855-680-9747 or the Human Resource department (850-877-5522).
When will my coverage become effective?
Newly hired full-time Employees insurance coverage will be effective the first of the month
following sixty days of employment. Payroll deductions for elected benefits will begin in the
month of your effective date.
When should I enroll in coverage?
You must enroll within 30 days of your full-time employment date. Employees who do not
complete online enrollment within the 30 day period will automatically be enrolled in the
Employer-paid Basic Life only. All other Benefit offerings will be declined.
How to Make Your 2014-15 Elections?
Directions to enroll online are available in the back of this handbook.
You must first register and set up a password before making your 2014-15 elections
Once you register you should:
- Verify your address and dependents.
- Verify your coverage tier (you only, you & spouse, you & child(ren) or you & family)
- Make sure you confirm your enrollments upon completion
- Designate beneficiaries for Basic Life and Voluntary Life (if elected)
What changes may be made outside of my New Hire
Enrollment?
Insurance plan elections may only be changed during the annual Open Enrollment (effective
August 1st) unless there is a qualifying change in status during the Plan Year. You must notify
the HR Department within 30 days of a qualifying event that occurs outside of Open Enrollment
if you want to make changes. A list of Qualifying events may be found on your Employee Benefit
homepage, login at myagprobenefits.com to view.
Is Evidence of Insurability required for Coverage?
On your initial New Hire enrollment all coverages are guaranteed issue regardless of health.
Some plans do have limited benefits for a period of time for Pre-existing conditions (please refer
of each plan summary for details). At future Open Enrollments some plans will require Evidence
of Insurability for acceptance.

Ag-Pro offers two Medical plan options for you and your family. The HSA
plan offers free preventive care and comprehensive medical coverage after
you meet your deductible. If you choose the HSA option, Ag-Pro will open a
Health Savings Account in your name and deposit $40 bi-weekly into your
account to help offset medical expenses while meeting your deductible. You
may also contribute additional money into your HSA account on a pre-tax
basis through payroll deduction.

The Copay Plan Option features rst dollar coverage for Wellness,
Physician Ofce visit copays, and prescription drug copays. Other services
are typically covered under your deductible and co-insurance.
Please refer to the Benet Summaries / Summary of Benets and Coverage
(SBCs) posted on the enrollment website for more details on the HSA and
Copay plans. Printed copies of the SBCs are available upon request.

Dental Insurance
Most people will utilize their dental plan at least twice a year; it is
designed to minimize your out-of-pocket expenses related to good oral care.
This is a voluntary plan offered by Guardian.
Premier Plan
Preventive Care Plan pays 100% for two cleanings per year, x-
rays and more. No deductible.
Basic Services The plan pays 80% for most services, including
llings, simple extractions, and anesthesia
Major Services The plan pays 50% for crowns, bridges, root
canal, and Periodontic surgery
Orthodontia 50% up to $1,000 for children
Deductible $50 once per year for Basic or Major Services

Basic Life Insurance
Your Guardian group life plan can provide nancial security for your
beneciaries if you die while an employee of Ag-Pro. Your beneciaries will
receive the amount specied below.
$15,000 death benet
Additional $15,000 for accidental death
The plan is paid for entirely by the Ag-Pro.

This is a voluntary plan in which you may elect to purchase additional life
insurance coverage on yourself, your spouse and your children. This is term
insurance: the premiums are low and the plan is portable. Coverage is
insured by Guardian
Employee
You may elect to purchase life insurance in increments of $25,000 up to
the maximum of $150,000.
Employee Guaranteed Issue: At your new hire and open enrollment only,
you may elect up to $150,000 of life insurance with no health questions.
Spouse
Your spouse is eligible for 50% of the Employee benet up to $50,000.
Spouse Guaranteed Issue: At your new hire and open enrollment only,
the rst $50,000 is guaranteed issue.
Children
You may elect 10% of the Employee benet up to $10,000 coverage per
child. The premium is a at rate, not per child.

Accident Plan
Guardians accident insurance is designed to help you pay out of pocket
costs such as deductibles, co-insurance, travel expenses, loss of wages, and/
or other non-covered expenses in the event of an off-the-job accidental
injury. The plan pays money directly to you and benets are paid
regardless of other insurance coverage you may have. Partial list of
Benets include:
Accident Emergency Treatment - $175
Accident Follow-up Visit - Doctor $50, up to 6 Treatments
Appliances - $125
Chiropractic Visits - $25/visit up to 6 visits
Dislocations- Up to $4,400
Diagnostic Exam (Major)- $150
Emergency Dental Work $300/Crown, $75/Extraction
Fractures- Up to $5,500
Hospital Admission- $1,000
Hospital Connement- $225/day up to 1 year
Hospital ICU Admission- $2,000
Hospital ICU Connement- $450/day up to 15 days
Initial Physicians Ofce/Urgent Care visit- $75
Lodging- $125/day
Occupational or Physical Therapy $25/day up to 10 days
Transportation- up to $500, 3 times per accident

The Premium Pre-Tax Plan gives you the opportunity to reduce your
taxable income by using before-tax dollars to pay for certain expenses. This
saves you money by reducing you taxes.
Premium Pre-Tax Plan
Any premium dollars that you have payroll deducted for your Medical,
Dental, Vision or Supplemental plans may be pre-taxed on your check.
This means that the payroll deduction for these benets avoid Federal and
State taxes. Therefore most people will save 21% to 37% in taxes on their
insurance premiums.
If you participate in the Premium Pre-tax plan you will be unable to change
your insurance plan elections until next years Open Enrollment unless you
have a qualifying event change as a change in family status.
There is no cost for this plan.
Life and Disability plans are not eligible for the Premium Pre-Tax plan.
