Female, 48 year old
Chief Complaint
Something entered at the right ear canal
since an hour before admission
Medical History

Something entered at the right ear canal
since an hour before admission
Previously the patient was sleeping at the
floor, suddenly something entered her right
ear, and moving inside her ear. The patient
went to DR. M. Djamil hospital and was given
sterile oil by physician.


Medical History
There was pain in the right ear
There was no effort to pull out the cockroach
There was no bleeding from the right ear
There was no history of discharge from the ear
There was no hearing loss
There was no fever, cold and cough
General Examination
General condition was good, composmentis
cooperative
BP: 120/70 mmHg
HR: 84x/mnt
RR :20x/mnt
T : 36.8

ENT Examination
Ear :
Right ear :
There was foreign body in 1/3 outer ear canal look
alike black colored insect without movement,
tympanic membrane couldnt be evaluated
Left ear :
Ear canal was wide, tympanic membrane was
intact, cone of light (+)
ENT Examination
Nose
Right nasal cavity : nasal cavity was wide,
inferior turbinate was eutrophy,
middle turbinate was eutrophy,
septal deviation (-)
Left nasal cavity : nasal cavity was wide,
inferior turbinate was eutrophy,
middle turbinate was eutrophy,
septal deviation (-)


ENT Examination
Throat
Pharyngeal arch was symmetric, uvula in
midline, Tonsil T1-T1 not hyperemic,
Pharyngeal wall not hyperemic

Working Diagnosis
Foreign body ( cockroach ) at right ear

Management

Extraction by forcep : succeeded
Irrigation of ear and applied povidone iodine at the
right ear canal
Evaluation :
Right ear :
Ear canal was wide, excoriation (-), laceration (-),
foreign body (-), TM was intact, cone of light (+)
control to out patient clinic if any complaint
