Summary
Patient X, 35 years old, who lives in Mangalcal Carmen Davao Del Norte was admitted at Davao Doctors
Hospital on July 21, 2014, 12:40 PM under Dr. Ledesma as her attending physician. 2 weeks prior to her
admission, the patient was noted to have urine dribbling occurring episodically associated with dysuria.
No medication was taken. A day after, the patient was noted to have vague hypo gastric pain with a 10
out of 10 pain scale, Persistence of pain prompted for consult at the OPD and was advised TRS: large
pelvic mass. Ovarian in right part. Dermoid cyst; L. may not visualized.Patient and was then advised for
surgery thus having the admission. The patient was given medications such as cefuxitin, ketesse, dolcet,
nalbuphine and metoclopramide.
