I would like to ask your permission to allow me to enroll PRACT2 and THS2 this 1

semester,
AY 2014-2015. This is in line with my PRACT1 which, to date, is yet to be completed and is a
pre-requisite of the aforementioned courses.

I will be finishing my PRACT1 course on or before June 30, 2014 which will include submission
of all requirements to my Practicum Adviser. In the event that I am unable to submit my
requirements on the mentioned date, I will automatically get a grade of INC (incomplete) for my
PRACT1. I also agree to drop PRACT2 and THS2 from my officially enrolled subjects for 1
st

semester, AY 2014-2015.
