Greetings!

I am Cristy Annie T. Elcano who visited the Office of the Registrar of Our Lady of
Perpetual Help located at Project 8, Quezon City last March 19, 2014 to request
a copy of my baptismal certificate to be used as one of the requirements in the
process of my upcoming matrimonial ceremony on June 09, 2014. However, one
of the clerks from the said office denied my requisition because of some minor
conflicts in my mothers name.

Since I am already here in Canada and all of my close relatives are also outside
our country, Philippines, I hereby authorize my future mother-in-law, Marieta dela
Cruz, to process the said documents. She will also bring with her related legal
documents to clarify the said conflicts in my mothers name.
