Well Services Safety

Introduction
Well service operations involves various risks. In order to ensure the
safety of people, asset and environment you must be aware of the
hazards and controls in place on work sites.
The hazards listed in this presentation are generic, and its your
responsibility to identify specific hazards related to each operation and
take all possible measures to prevent injuries, failures and damage to
assets and environment.

Hazard Identification
Hazard is anything with potential to cause harm
to PEOPLE, ASSETS and ENVIRONMENT
Hazard identification is the ability to LOOK,
SEE and OBSERVE the situation
Often we LOOK but we dont SEE
We SEE but we didnt OBSERVE
We should LOOK TO FIND OUT
SEE TO KNOW
and OBSERVE TO UNDERSTAND

Major hazards on well sites

Deadly H2S
Attend the H2S/ BA training course annually
Carry an H2S detector whenever you go to site
Carry a communication device to contact the emergency service
Check the H2S concentration frequently
Check if the windsock is installed for wind direction
Ensure BA sets are ready for use at anytime
Ensure all connection are perfectly tightened
2.1
Chemicals
Use proper containers and tanks suitable for the purpose
Have MSDS on site, even for sample chemical (including diesel)
Read MSDS for each chemicals
Label all chemical containers (big and small)
Follow proper procedure for mixing
Dont use open tank for pumping hazardous chemicals
(e.g. displacement tank)
Add water in the tank before filling with acid
Use appropriate PPEsfor chemical handling
Ensure the safety shower is available, connected and working
Have eye wash solution and first aid kit on site
Dont mix incompatible chemicals
No ignition sources (flames, sparks) near flammable chemicals
Have secondary containments wherever possible
2.2
Pressure failure
Supervisors to attend Well Control training
Use only certified and integral pressure containing equipment
e.g. BOP, Risers, WH X-Over, Check Valves, Pumping Irons, Air tanks etc
Test the pressure containing equipment prior to job
Have PRVson high pressure lines (The outlet of the relief valve must be
positioned so that it is pointing away from the equipment and personnel)
For tests with liquid, air must be removed
Tests must be conducted with liquids, whenever practical
Secure the lines with proper whip checks
Check integrity of the seals before rigging-up
Barricade the equipment being tested
Shield the controls and operators
Evacuate all unauthorized personnel from vicinity
Have signs reading Danger-High pressure test in progress Keep out
The working pressure must be marked on the front of the fluid end
The indicator bars of high pressure hammer valves must be painted a
different color from the valve.

Fire hazard
Carry out proper maintenance of equipment as per OEM
Proper electrical wiring on equipment
Remove ignition sources from flammable chemicals
No open flames or other ignition sources near wellhead/ choke manifold
Have emergency shutdown on equipment
Use only intrinsically safe components near wellhead
Only Zone-2 certified equipment on well site
Dont store flammable chemicals exposed to sun light
No steel hammer on wellhead, choke manifold, and production lines
Maintain good housekeeping
Have fire extinguishers on site
Have communication device and emergency equipment on site

Spillage (Environmental hazard)
Inspect the tanks/ tankers before storing chemicals
Create bunds around acid tanks on site
Lay plastic sheet where the chemical is being mixed
Reduce water spillage
Connect pumping irons and hoses properly
Ensure that the CT tubing is in good condition
Have drip tray and drain plugs on all equipment skids
Use provided disposal lines for cementing pump cleaning
In the event of large spillage immediately evacuate the area
And Inform your supervisor immediately
Report any potential spillage or failure of containments

Raised loads
Only qualified crane operators and riggers allowed on site
Discuss the job before starting lifting operation
Assign a Signal man
Signal man to wear reflective jacket
Use certified crane and lifting devices (Have certificates on site)
Have SWL printed on all lifting equipment
Inspect the slings and lifting points before the operation
Have injector legs fixed on steel bars (not on cellar wall/ mesh)
Keep clear if you are not involved in the operation
Barricade the area
Hands away and stand clear before lifting the load
Use taglines to guide the loads
No lifting during heavy wind, sandstorm, thunderstorm etc.
Secure the loads before mobilization/ lifting
Ensure no loose items are in the basket (LSE items?? See next slide)

Moving vehicles
Parking at the designated areas only
Be alert of moving vehicles and equipment
Use stopper chocks for parked vehicles
Connect a tractor head on CT unit trailer on site
Ensure all pilot lights are working
Use signalman wearing reflective jackets while moving long vehicles.

Electric shock
Ensure that equipment, vehicle, and crane are not in contact with
overhead lines
Never touch a fallen overhead power line
Stay at least 10 feet (3 meters) away from overhead wires
If working at height check the area before starting work
Never try to repair electrical cords and appliances
Only qualified electricians to repair electric appliances
Use weather proof and industrial fittings for site use
Ensure all electrical wires and appliances are free of defects
No electrical equipment at Zone-0
Follow proper color coding
Electrical tools must have 3 wire (grounding) cord and plug,
excluding double insulated tools

Pinch points
Ensure guards are in place on all rotating equipment
Handle the tools and equipment carefully
Dont modify of remove machine guards
Be careful while opening and closing tool boxes, vehicle bonnet etc.

Manual handling
Bend the knees, but not beyond a right angle
Keep the back straight, but not vertical
Lift using the strong thigh and calf muscles
Keep the centre of gravity of load and body in line with the feet
While carrying, clasp the load close to the body
Keep elbow close to the side
Ensure you get a good grip of the load - use the palms
Position the feet correctly
For team lifting one person must take charge of the operation
(Note: The weight of to be lifted should not be more than two thirds of the sum of the
individual capabilities of the team members (Men: 25kg, Women 16kg)
Do not turn the body or head while lifting. Lift, and then pivot on feet
Do not jerk or snatch. Slowly accelerate the load
Do not use the weak back muscles to lift.

Extreme cold
Use proper PPE for Liquid Nitrogen Handling
Dress warmly and stay dry during cold weather
Avoid Exertion if you have heart decease or high blood pressure

Extreme hot
Know signs/ symptoms of heat-related illnesses
Monitor yourself and coworkers
Block out direct sun or other heat sources
Take regular breaks and stay at shades
Drink lots of water
Wear lightweight, loose-fitting clothes
Avoid caffeinated drinks, or heavy meals
Use hard hats and UV protection goggles

High noise level
Turn the noisy machine around
Do proper maintenance and lubrication of the noisy equipment
Use ear PPE (ear plugs or ear muffs)
Use equipment with no or very little noise
Have silencers on engine exhaust
Have absorptive materials (engine mountings)
Have screens wherever possible
Stay away from noisy equipment if possible
Reduce the time of exposure to noise

Slip, trips and falls
Use proper footwear
Have proper steps and handrails for CT Cabin
Use Safety harness while working at height
Have proper working platforms for work at height
Clean up the spills immediately
Remove obstacles from walkways
Secure mats, rugs and carpets that do not lay flat
Cover cables that cross walkways
Keep working areas and walkways well lit
Take your time and pay attention to where you step in

Deep water
Be careful while walking over the water / chemical tanks
Dont go to the shore unnecessarily
Have HUET / Sea Survival training for offshore operations
Follow all offshore rig instructions on water safety

Pre-job preparations
Job Supervisor is responsible to:
Comply with all QHSE Standards
Review the job program
Conduct a Site Visit
Collect details of the well (Type of well, Type and size of wellhead connection,
Pressure rating of wellhead, H2S concentration)
Propose a crew
Conduct a meeting to discuss job preparation
Prepare equipment and chemicals (including safety items)
Consider safety standards while designing the job
Review and approve mobilization arrangements
Check equipment certification
Review contingency plan
Complete the documentation

Transportation Safety
Drivers should have valid and appropriate driving license
Use vehicles / transport equipment suitable for purpose
Vehicle used for Sand to carry, One spare wheel, a shovel, wheel
chocks and jack support.
Ensure there is enough fuel in the vehicle for journey
Carry out pre-journey inspection (Fuel, Oil levels, Jack, Seat belt, Tires, Body, FA kit,
Fire Extinguisher, Lights, etc)
Carry a communication device; Dont use mobile phone while driving
Ensure loads are secured before move
Dont leave chocks, wooden bars and chains on trailer after offloading.
Carry a copy of MSDS for chemical transportation diesel included
Know emergency numbers for any emergency assistance
Contact the base support team regularly
Drivers and all passengers should use seat belt
Smoking is not allowed in the vehicle
Activate drivers monitoring device if equipped

Discuss the following:
Outline the job procedure and define the pressure limits
Pressure testing procedure
Job hazards
Emergency alarms, assembly area, wind direction and escape routes
Define emergency team & personal assignments
Communication procedure

Now you have an idea about the major hazards on well sites, and the safety
precautions to be taken to combat the involved risks. Also you are prepared
to LOOK, SEEand OBSERVEthe hazards.
Well, now its your turn to confirm whether all hazards are identified and
eliminated, minimized or controlled the related risks.
There is an easy way to do that.
