    Complete this form and email it to shelleycancer5k@outlook.com or
    place it in the drop box located inside the Broulims in Shelley. When
    emailing the form please rename it to your first and last name to ensure
    that all forms are received. Registration forms will be due Thursday
    October 16. There will be a registration for anyone who has not
    registered on the morning of the race.
