Undersigned Chief of Police of Leyte, Station, Leyte-Leyte, after duly sworn to law, accuses Don-
don Ancero alias Don, of legal age and resident of Brgy. Poblacion Leyte-leyte for the crime of Serious
Physical Injury committed as follows:
That on or about 8: 00 in the evening, September 11, 2014 at Brgy. Poblacion, leyte-leyte and
within the jurisdiction of this honorable court, the above-named accused, with deliberate intent, did and
there, willfully, unlawfully and feloniously attacked, assault and about one Mr. Richard Delima with a
full force fistio blow in his face, thereby inflicting upon a result of Serious Physical Injury which requires
him to undergo medical attendance from one month to three months and incapacitated him that to
perform on his customary labor for the same period of time. To damage and prejudiced of herein Mr.
Richard Delima.
