HOSPITALS/NURSING HOMES/2013-14, Dated 15.01.2014, at Kolkata.

Sir,
I, Shri Atindra Narayan Chaudhuri, a retired Senior Technical Supervisor o/o. The Deputy General
Manager (Q.A.), B.S.N.L., EP - GP Block, Sector V, Salt Lake, Kolkata 700091, bearing the Regn. No.
BSNL Retired/033-QA/KOL and PPO No. KTK/CCA/IDA/SA/QABC/786, dated 13.07.2005, have been
suffering from a prolonged spinal affliction since retirement, and consulted many expert physicians to get
relief from it.
Presently, Im under the treatment of Dr. Amitabha Chanda, MS, MCh, Masters in AO Spine, attached to
Fortis Hospitals Limited, 730, Anandapur, Kolkata 700107. After thorough medical examinations &
assessment of my ailment, he advised me to undergo a neuro-surgery immediately, for which Ive decided
to take admission in Fortis Hospitals Ltd. On 13.09.2014, failing which the doctor has told me that I may
get paralysed permanently.
Under the stated circumstances, I beg your kind attention to this matter and sanction the medical
reimbursements approved by the authorities as per the BSNL guidelines. In this connection, photocopies
of all relevant documents are annexed herewith for your kind perusal.
I would also request you to kindly send an official representative for verification as soon as possible. My
