affirm and solemnly declare that the vehicle Make ________________________Chassis
No______________________ Engine No _____________________ Model ________________
cleared in Amnesty scheme vide SRO 172(I)/2013 dated 05.03.2013 is neither a stolen vehicle
nor its Chassis Number is tampered with. I further declare that the vehicle was presented for
examination at the time of assessment at the station where it was cleared from under the
Amnesty scheme.
I solemnly declare that in case any thing stated above is found untrue, I will surrender the
vehicle to the customs authorities on demand without entering into any litigation or taking any
legal recourse whatsoever.
