7.1 Utilize conceptual frameworks to guide the processes of assessment,

intervention, and evaluation

Complete Discharge Planning Assessments on all new patients that are admitted to the
unit, then type assessment into Midas.

For patients that are readmits, obtain in patients own words why they have returned,
then document in Midas.

Complete Progress Notes, Discharge Notes in Midas.

Maslows Hierarchy of Needs and Person-in-Environment are the two most suitable
frameworks that guide the course of services for patients at CFVMC.

Practice Behaviors
7.2 Critique and apply knowledge to understand person and environment
Identify a patients needs, understand Physicians Orders, communicate with patients
and staff to have a better understanding of patients health and health care needs, and
abilities to follow-through on discharge orders to ensure.

Understand how a persons environment can have an impact on their health and
healing.
