Fibric Acid Derivatives

MOA:
Peroxisome Proliferator- Activated Receptor-alpha agonist (PPAR-).
Stimulate fatty acid oxidation.
They transcriptionally up-regulate LPL, apo A-I and apo A-II, and down-regulate apo C-III, an
inhibitor of lipolysis.
EFFECTS:
Decrease secretion of very-low-density lipoproteins(VLDL)
Increase lipoprotein lipase activity
Increase high-density lipoproteins (HDL)
CLINICAL USES:
Used to treat hypertriglyceridemia.
Low HDL cholesterol
TOXICITY:
Nausea- most common
Skin rashes- common with gemfibrozil
When used in combination with reductase inhibitors or niacin, the fibrates significantly increase
the risk of myopathy.
Hepatic dysfunction
Cholestasis
GI distress

Clofibrate- DOC for Type III hyperlipoproteinemias
